Who can participate ?
This program is designed for older adults with multiple chronic conditions and connects participants with a personal health and wellness coach or counselor to improve physical, emotional and social well-being. This program uses motivational interviewing techniques and validated assessment tools to create health action plans and accountability. Through the use of problem solving strategies, participants clarify goals, responsibilities and activities as they work toward health-related behavioral change.
Participants must be willing to answer a health questionnaire and be open to behavioral change and accountability.
Does the program replace existing programs and treatments ?
The program does not replace any existing programs or treatment. Enhance Wellness complements formal healthcare services, and is designed to work seamlessly with other evidence-based programs.
What can I expect ?
Research has shown that participants in Enhance Wellness can decrease the length of hospital stays, lower their use of psychoactive drugs, alleviate symptoms of mood disorders, and develop a greater sense of self-efficacy.
From February 1995 to June 1996, researchers conducted a study (formerly the Health Enhancement Program) and followed 201 disabled adults, aged 70 and older. The results (later published in the Journal of the American Geriatrics Society) showed that the program led to improved ADL functioning in those who were disabled and thereby offered a promising strategy for limiting or reversing functional decline in disabled older adults.
Details of the program
Enhance Wellness is an individualized health coaching program for older adults with multiple chronic conditions. A one or two-person team works with the participant to motivate behavior change over a six-month period. Enhance Wellness is made up of three parts:
- Screening: The Enhance Wellness staff member and the participant meet and review a detailed health questionnaire. This questionnaire helps to identify personal strengths and risks.
- Planning: The participant, with staff coaching, creates an action plan that focuses on areas he or she wants to work on. The plan is shared with the participant’s physician.
- Action: As the participant moves into action, a health professional/ coach offers encouragement, feedback and monitoring. The health professional helps with problem solving, health education, and referral to support groups and additional services such as individual and family counseling, if needed.
Sessions usually last 30-60 minutes with the exception of the initial session which can last up to 90 minutes. During the initial session a health risk assessment is given and from that a participant determines the health behavior that he/she would like to change. The participant decides what to work on, with whom, where and how often. From this information, an individualized Health Action Plan is developed.
Sessions generally occur once a month for a minimum of 6 months.
